MUL - Program Service Specialist III

The Minneapolis Urban League’s Social Wellness Cluster is seeking a Program Service Specialist III to provide case management services to clients living with HIV. This individual works independently w/minimal guidance performing duties which include the following:

Does client assessment and evaluation
Makes referrals & links client w/service providers.
Maintain caseload of 25
Transports clients to scheduled appointments.
Conducts home visits in connection w/ case management.
Maintains client files in accordance with DHS standards
Recruits clients for Case Management services
Other duties as assigned

Knowledge of HIV/AIDS and resources available to target population. Ability to maintain detailed & accurate case notes. Strong computer & verbal/written communication skills req. BA degree + 1 yr exp. or professional certification in targeted area, or HS diploma & five years experience in counseling, case management or related field, w/3 of those years working w/targeted population (HIV/AIDS clients).
